Characters difficulty classification question

Guys, Do you always recognize the difficulty of the encountered enemy in a computer game by there visual appearence? I think thats easy to accomplish if you are doing a sci-fi shooter where you can just make the monsters look meaner. But how can I accomplish that when all the enemies are normal human beings? How is that accomplished in max payne for example.

We are currently developing a game where your man enemies are normal human beings. Is it acceptable to have the same model carry different weapons and have different health levels through out the game?

How can I classify my characters into weak, moderate and strong if most of them have the same visual appearance (ie all human)? Thoughts and ideas are welcomed :)

I'll go out on a limb here and say audio will be your key. Changing the music to be more ominous or high energy would affect the mood of the combat. To piggy-back off your reference to Max Payne, whenever you are about to fight a "boss", the music changes to a more high-tempo rhythm and the "boss" spouts something signifying a tough fight is on your hands. If you have a good camera system and cut scene class, you can even separate your strongest enemies with quick cinematic.

Give them bigger arms, better weapons, maybe armor, and make them yell.
