Holes and missing faces

Hi guys,
I have an ongoing problem that's giving me fits. It seems like half the DIFs I export are missing a face somewhere. I'm using 3D World Studio currently, but I've had the same problem in Quark.

The problem happens most often in hollowed-out shapes like tunnels and walkways, regardless of whether I'm using the Hollow command or carving with brushes.

Any suggestions on the cause of the problem, and how to remedy it?

Are you using Map2Dif Plus?
Does your editor report any errors on non-convex brushes?

Yes, I'm using plus, and no brush errors are reported -- everything comes back clean, but the end result leaves much to be desired.
