The image upload page should have a full form on it with about three boxes for entering data. Have you tried looking with other browsers or on other computers to make sure it's not some weird glitch on your end? Just checked and they show up for me (running 1.5.0.1 on my end).

Still no form - very odd, can you e-mail me a screenshot and also the source of the page, as well as the URL that you entered to get to it? beng@garagegames.com

You should be able to do links with query strings in them, by doing [http://projects.blender.org/frs/?group_id=95].
