Ok I have come to;

And of course I have some question about it.
What are am doing now??
Am so blank
Can't I just say: All in know universe or something. For if I must tell about every city, or something, that is to BIG. Then I must tell first about London in anno 1900, instead. That I can do. But how do you tell about a city as an layout???

Depends on your game, but for my RPG, I made a general map showing villages and major structures across the landscape, then have detailed narrative describing the major locations that are shown on the map.

Overview
Asparon is a humid jungle world. Almost two thirds of the planet are covered in thick vegitation, with the remaining one third covered in water. The animal life on the planet is suitably adapted to this environment, with many of the beasts being experts at camoflage.
The WALTA units are not well suited to this envirnment and the engine intakes tend to get clogged with leaves and debris from the many plants and vines that the machines need to walk through.
World Layout Detail #1: Donnovans Rift
This is a large valley carved into the surface of the South Continent. This is where the first Asparon missions take place and is also where the forward base has been established (see Asparon Forward base images in appendix 15). Blh blah blah......
You basically need to describe all the important details of the world so that someone coming to the document with fresh eyes, will understand what your intentions were and what significance the world and the environments have.

Actually this section should contain an overview text description of the world in general and detailed descriptions of the various aspects of the world and refer to images in the apendix if necessary.Example:

Ooooooh, ok thats FUN. Thanks guys for all help. Thas more understandable for me. Am already omw to made map of London anno 1900. So I will not get lost in my world. :)

I draw out what the entire world is going to look like in hte game.
That way everything is defined and nothing is left to guessing.
